Tennessee Prisons Chief Resigns After Failed Execution of Christa Pike

Tennessee's prisons commissioner, Frank Strada, has resigned days after the state's failed attempt to execute death-row inmate Christa Pike. Governor Bill Lee announced the resignation on Saturday, October 3, 2026, and said Strada will step down this month while an independent review investigates what went wrong.

Pike survived two doses of the lethal drug pentobarbital at the Riverbend Maximum Security Institution in Nashville on Wednesday, September 30. Her lawyers said in a court filing that she was left unconscious, intubated and on a mechanical ventilator, with hospital staff working to clear the drug from her system.

A second failure prompts a freeze

This was Tennessee's second failed execution attempt this year, prompting Lee to halt all executions in the state for the rest of 2026 and order the independent review. Pike's attorneys attribute the failure to problems with the IV line, which they say caused the drug to enter her body rather than her bloodstream, and accuse officials of ignoring doctors' warnings about her small veins and thrombocytosis — claims the state has not publicly answered.

Strada, who took over the department in January 2023 after serving as deputy director of Arizona's corrections department, said his personnel followed proper protocol but that stepping aside was "in the best interest of Tennesseans as the independent review moves forward." In late 2024 he had announced Tennessee would use only pentobarbital for lethal injections.

The underlying case

Pike, 50, was convicted three decades ago of first-degree murder for the torture and killing of 19-year-old Colleen Slemmer in 1995, when Pike was 18 — a fact established in the official conviction record. The botched execution and the resignation of the state's top prison official have reignited the national debate over the death penalty.
